PLEASE FOLLOW THESE INSTRUCTIONS IN ORDER TO BE CLEARED TO VOLUNTEER IN OUR CLASSROOMS OR ATTEND FIELD TRIPS

  1. CONTACT YOUR PHYSICIAN FOR TUBERCULOSIS (TB) CLEARANCE 
  2. COMPLETE OUR VOLUNTEER REQUEST FORM 
  3. COMPLETE THE LIVESCAN FORM for an outside agency.

ACCESS OUR FORMS AND OBTAIN MORE INFORMATION, PLEASE VISIT OUR PARENT PORTAL PAGE FROM OUR DISTRICT WEBSITE AT MDUSD.ORG AND CHOOSE OUR VOLUNTEER INSTRUCTIONS LINK

Take the livescan form to the outside agency of your choice and complete the fingerprinting process. The agency will charge a fee for this service. The results will be sent electronically to the district office.

BRING YOUR TB CLEARANCE,  AND VOLUNTEER REQUEST FORM AND YOUR CURRENT VALID I.D. to Human Resources at the district office, 1936 Carlotta Drive, Concord.

CURRENT VALID I.D. REQUIRED

ONCE THE DISTRICT OFFICE HAS RECEIVED THE ABOVE REQUESTED DOCUMENTS FROM YOU AND YOUR FINGERPRINTS HAVE BEEN CLEARED THROUGH THE DEPARTMENT OF JUSTICE (D.O.J.) YOU WILL BE ADDED TO OUR LIST OF VOLUNTEERS.
